You can follow this tool:

This tool can generate CAP for jc2.1.2, 2.2.1, 2.2.2, 3.0.3, 3.0.4 and 3.0.5.
You can load jc3.0.3 CAP for your card.

Could you please let me know where did you get NXP J5C145 java card from? I need exactly the same java card with same specifications but I am not able to find one.
It would be great help if you can direct to some link where I can buy this Java card from.
